Cast when damage taken is a trigger-support gem. It triggers skills automatically when the player takes specific types of damage, and does so for as long as it’s socketed in an active skill slot (even if that skill has been swapped out). The supported spells are cast according to which type of incoming damage they’ll counter: physical spell, fire spell, cold spell or lightning spell.
When triggered by any other kind of hit from any source – including DoT effects like poison – then nothing happens.
Rationale: One way to mitigate pvp survivability is through self healing and casting certain skills on oneself after taking some amount of elemental damages will help with this goal. Cast when Damage Taken can be used as one such tool.

The gem is used with a low level trigger support gem such as Molten Shield/Puncture Shot Support since these will apply at least some amount of damage before triggering CWDT gems on caster destruction. This ensures that the first line against elemental attacks have already done their job when a spell is triggered.
This system also has the advantage of never triggering on damage done to enemies, and only ever triggers when you take physical or elemental damage from an enemy – so it won’t trigger on things like taking spike-damage against porcupines in some maps, for example.
Molten Shield/Puncture Shot Support gem is used with CWDT because these will apply at least some amount of damage before triggering CWDT gems on caster destruction. This ensures that the first line against elemental attacks have already done their job when a spell is triggered. Molten shield can be replaced by any low level skill such as:
Rejuvenation Totem(level 20), Clarity (level 23), Flame Dash (level 28), or Enduring Cry (level 35).
If you want to use a high level skill such as Glacial Hammer, Arc, Ball Lightning, Volatile Dead/Ice Spear from the Ancestral Elementalist Ascendancy and still be able to trigger your CWDT gems against physical attacks then you need Culling Strike Support gem. This will allow spells like this to damage enemies before they are killed by a hit from an enemy with CWDT. It’s important not to over-level these skills because it can cause lag issues when using them for triggering on cast of spells support gems. For example: Glacial Hammer is only unlocked at Haku tier so must be used in conjunction with Cold Penance Jewels if wanting access higher than level 28.
Levels should also correspond with the skill level. For example: Skill Glacial Hammer is unlocked at Level 18, so it’s best to use a CWDT gem trigger of 25% or less on cast when damage taken – lvl 20%.
Again, this should only be done with high-level skills that don’t require more than one click per attack and have no cost. This will ensure they are triggered reliably every time you take damage from an enemy hit by your spells support gems in your Cast When Damage Taken line. If using low levels like say level 15 then there would not usually be enough spell power for triggering them reliably unless supported by some other hard-to-get mods such as Elder supports/Cold Penance Jewels/Malachai Buffs.
The rule of thumb for the amount to use as a trigger on Cast When Damage Taken is 25% or less, so you should lower your level if needed. Level 20 will work best when using skill Glacial Hammer which needs one click per attack, and it’s only unlocked at level 18. The corresponding gem levels would be Gem Quality: Required – Ilvl 36-40 (Roughly) and then Trigger with Mana Cost: Reduced by 75%. This ensures that CWDT triggers every time damage taken support gems are activated in our Cast When Damage Worn Line Linked Items section below.
There are some skills like Cold Penance Jewels can activate them reliably all the time but they’re hard to get from trading and expensive to buy.
The new Malachai Buffs will trigger the spells linked to when damage taken support gems, but they’re hard for them too since you can only get from changes and are pretty rare also.
Trigger with Mana Cost: Reduced by 75% specifies that each time CWDT triggers it consumes no more than a quarter mana pool so if your intention is to reserve most of your mana then some other option like Trigger with Cast When Damage Taken Support Gems Leveled should be used instead because it doesn’t have limits on how much power goes into triggering skills in this line link items section below.
The beauty about these options listed here is that there’s versatility in what you want or need from one another without having limitations.
Trigger with Cast When Damage Taken Support Gems Leveled: reduces the spell level of CWDT to levels supported by your gems. The max are 20/30/40 for lvl 18, 19 and 20 gems respectively. This option is a good choice if you want high-quality spells but don’t have enough mana capacity or other slots in which to put them.
Auras can be cast without cost when taking damage because they’re passive skills so this could help increase their number of casts per second as well as increasing the duration that they last during combat since it’s not using up any power from your pool at all times like most of these do here on weapons section below.
Trigger with Increased Duration: specifies how long the buff lasts.
Trigger with Increased Area of Effect: specifies how large an area it affects (defaulting to a radius of 0).
Trigger with Reduced Duration and increased effect per gem level: reduces the duration by 20% each time you use this type of trigger, but increases the strength at which they work for every spell level in that link. For example if there are two gems links then one says “Cast on damage taken” while another is “increased critical strike chance”. If both have quality levels, say 15%, then cast when damage taken will last twice as long as increased critical strike chance because its percentage reduction is applied before any other multiplier’s percentages are calculated.
Spell Totem Support Gem Leveled: causes the totem to be invincible and will attack enemies in range. Multiple Totem Support Gem Leveled: causes the spell to summon more than one totem at a time, but it does not affect how many projectiles are fired from each of them. Spell Echo Support Gem Leveled: increases the number of casts that occur by every level (defaulting to two). Quality on Gems Supported by CWDT: reduces cast when damage taken’s cooldown for 20% per quality level on all supported gems. Trigger with Increased Duration and reduced effect per gem level: increases duration, but decreases strength for every spell level in that link.
